Does it happen to all your games or just Black Ops 2? After searching around, some users have said it can either be a problem with the disc drive or something not working properly in your cfw. I'd personally try to downgrade your PS3 to 3.55 through your Rebug cfw and reinstall Rebug 4.70.1? Don't quote me on this as a working solution but it's what I would do.

Only bo2 i think it might be a cfw problem
