This is a common error in COD4 and has nothing to to do with custom patches. It usually happens when there are 2 or more PS3s that play COD4 under the same IP. It's a network issue. Try these steps and hopefully your problem will be solved.
1. Unplug your PS3
2. Restart your router.
3, Wait about 5 minutes and then turn on the router and plug in your PS3 again.
4. Turn on PS3. The game should be fine now.

Your right and your wrong. I got one question to the guy. Are you on BT broadband? If you are then thats your problem. BT Broadband is known to have this issue. ABout 50 people i know who get this is with BT. Yes your reason is right but still its BT.
