I use Choco's Ultimate Private Patch

Delete the patch, reinstall it, and replace the patch again.

I had the same problem in the past. This is what you need to do.
First go to FIND GAME (this is were you can select game types like team deathmatch or free for all).
Now instead of being able to select a game type it should be stuck saying downloading game settings or something.
What you need to do now is turn off your internet box wait a couple of seconds and turn it back on.
Once your internet is back up and running go to FIND GAME again and it should ask you to sign in.
Sign in and you should be able to play online or use private match with no broblems.

When I first used a cod4 mod menu I had the same problem all I did was delete the game update/patch then reinstalled it put the patch back on and that dident work so I did so research and found out that all I needed to do was reset my router bam doing mod menus
