I've never had this happen on another game. I used turtle beach headphones and with black ops 2 multiplayer, sometimes the voice actor announces something and the game overall becomes and stays louder.

Not exactly that per say, but I have something very similar. It happens to me on my Ax Pros and my buddies headsets where the game volume just changes anywhere from low to high every time I start the game (with no changing in settings of the game or headset.) I will sometimes have to put the volume close to max and then the next time I start the game I'll have to lower it majorly, it's very confusing.
