Think found the cause all it took was bunch of games rewatching them and watching some old beta gameplay.
Appears to be down to idle sway plus flinch.
Flinch is toned down compared to the beta you get little bit of it but enough to throw you off if you don't control your gun.

Small number on average caused by flinch/idle sway rest was caused by poor awareness and positioning.
Something else may be going on but appears that is having an impact on average 2-3 death a game for myself. Compared to the beta which was 7+.
