It probably hit him in his transition from standing to prone in Second Chance, perhaps that's why it didn't kill him.

Yeah, like Altair said, it might be that it's hit him as he's going into last stand. But the game's still glitchy and sometimes you get hitmarker when the tomahawk gets stuck on an invisible barrier.

Maybe think in life a tomahawk wont kill you if hit in the foot.
