This hasn never happened 2 me, but did you check the other places 2? BTW there was one time when i was playing split screen the player 2 didnt have those lamps in that map that show u the location of da box...

This randomly happens after fire sales. Normally if you hit the box where the normal box should be (in your case, the dressing room) during the next fire sale, it should go back to normal.

yes we checked every where, and those dam zombies werent dorping any powerups.
