I get directly behind him, ADS on his head, and boom. Hitmarker. Had to take a freaking 2nd shot to kill him. About 2-3 mins later, he's back on the minigun again. I aim at the back of his thigh, ADS, boom. OSK. Near the end of the match, I catch him there again, ADS on his head, and this time it's an OSK.

If this isn't proof that there's something in the game that messes with bullet damage, then I just don't know anymore.

Same thing happened to be tonight, i was playing tdm in estate, found a guy upstairs in the house sniping out to the boat house, i got close to him and hip fired, hit markers, now i never ads with a shotty, and i usually always get a one hit kill, i have the grip.
and also how people can one burst me with the m16, without stopping power, but i have a hell of a time doing it. though the main thing that ticks me off is the lag issues, i wish they could somehow put all of the laggy players together, because im sick of a good round getting ruined because of lag.

Was he using painkiller?

Region-locking servers or allowing dedicated servers would greatly cut down lag. Too bad mw2 doesnt support either.

could depend on distance possibly. i noticed and might be wront that if u shoot while u are walking, i got a one shot kill. when im usually standing still it takes me 2 shots. i gotta do some testing in split screen tho.
