I would like to share an easy way to get the date of birth of your PSN account because I was struggling with that problem for while !!
anyway what you need to do is:

1. Connect your PSN account to any ORIGIN games, for example, FIFA, BATTLEFIELD... etc.
2. login to your ORGIN account online from any browser.
3. Click on My account on top of the page then Click on "About Me" and you will see the Date of birth under basic information which the one you insert when you made your account

Or you could just use your real DOB, so unless you have severe Alzheimer's it's not something you'll readily forget.
