Put your tool and all its files in C : \Program Files (x86 if you have 64 bit windows) \SN Systems\PS3\bin then load up from there connect and attach

it happened to me before,
if u got everything in the right place and still cant attach, then follow this:
delete the update that u installed from HDD000/game/(ur game region BLES/BLUS).
open the game and install the updates from psn form ur PS3. ( it will probably download last 3 or 4 updates). Wait till it finishes.
then use ftp server and extract the EBOOT from the file HDD000/game/(ghost region)
use TrueAncestor SELF Resigner v1.85 to build new DEBUG EBOOT. After it finishes.
place it to the same folder u took the eboot from and change the original eboot to EBOOT.ORG in case as a back-up.
Im sure after that, if u placed everything in the right direction, it will work perfectly.
