You are using details such an address which are currently or have previously been linked to an adsense account.

If you search for adsense so you wont get it to youtube.
only way to get ads to youtube is that Google Team contact you

Well maybe is someone in your house ? or a friend who knew your address and put it find out
Or remove the address if possible
