Two separate photos and some clone tooling.

As EE said, you need a front shot and a side shot of who ever you're going to use. Then pretty much put a layer mask over the front shot and pentool the chin, lips, nose and forehead from the sideview to get this look. Then simply replace the nose in the front shot with the side shot one to give it that illusion If any of that makes sense xD
