Install it into your mw2 game directory

I personally don't use any patches unless I'm testing for a friend I'd just use an rtm tool or an sprx menu if I were you. But I'd recommend aggression v1 or phantom v4 pretty much all the new ones are the same

Yea ive been trying to find good sprx menus for bo2 or just about any other game. But am having no luck. And i have no clue how to use tmapi
