No you do not need to run a mw3 backup for it to work. As long as you have CCAPI installed, then your good. You can rtm without a modded eboot, but i suggest getting a force host eboot. I can help you with it if you want. If so, what's your skype?

It's a very easy program to use and includes mods for your eboot.
