Are you on a CF/OFW, are you uploading new trophies or getting trophies you already had onto a new ps3. A lil more detail will help get a fix.

OFW, didn't have really any of the trophies, but I downloaded a modded BA Rank save from here... I think that's the problem, but I don't know how to permanently fix it.
