It largely depends on which PS3 size you have. If you have 160gb PS3, there will be less available slots because each account slot requires memory. I have a 320gb PS3 slim and it holds the maximum of 30 accounts, maybe get a new PS3 with higher memory or upgrade the hard drive. If not that then it maybe because you are using CFW.

64 is the lifetime amount of accounts a PS3 can hold. So every time you add another account to your PS3, even if you delete
that account, it'll still +1 to the total amount you've had on the console.

As far as the max at one time, I strongly believe its 16.
