Did you eject your flash drive the way it is supposed to be?

Same thing happened to my friend when i was trying to do it to his ps3. Mines work cause i updated the ps3 via internet and says 4.20 (for shop) and went ahead and install. It does not work on ofw 4.20 cause for some reason it gets an error.
