deleted the trophy data and try again
that happen to me on BO2
after I did that it worked =0)

it says Trophies failed to initalize please exit from the game

you need to delete the trophy data from GOW and try again

Who’s Jim Erased?
Delete your GOW game data and delete the trophy in: dev_hdd0 > home > 00000XX > trophy > GOW TROPHY FOLDER

Then open the game again, now it should work fine.
