It disables syscalls for your system, I believe. You'll be fine using that option, shouldn't cause any problems. It is used to help prevent detection by Sony.

Like insight has said, it basically disables the CFW and make the ps3 looks like your OFW.
Also, clears history of you running homebrew
Basically tricks PSN/SEN into thinking your running a legit PS3.

ok bro i have problem with disable cfw that is if i use it i cant play games through multiman when i choose the game from my external hdd and when im back to XMB the game didnt show up and i cant start the game why ??
please i need answer

The issue is that you need to run MM (Multiman) and mount the game before running PSNpatch
So what I need you to do is run MM
Go to MM settings
Go all the way down to BD-Emulator :You must login or register to view this content.
ENABLE it
Then restart MM
Mount the game
And then run PSNpatch

but if i use disable cfw and play game on disc and after i quit the game i cant use multiman even if i turn off the console the only way to fix this is to delete multiman and install it again and this is really annoying. please how i can fix this

Ok listen brother
You need to always use MM before PSnpatch
If you do it the other way around PSNPatch before MM
PSNpatch will disable BD-ROM emulator!!!!
And it will stay disabled until you will have to enable it.
Therefore if it is disabled BACK UPS wont mount.
SO please read my previous statement!!!!!!!!!!!!!!!!!!!!!!!!!!!!

PSN Patch, disables your Custom Firmware SystemCalls. In a manner it makes it to where your system shows up as a OFW. It makes it harder for Sony to tell if you're on a Custom Firmware console and hides it to an extent. It also spoofs multiple things that could be handy along the way of modding certain games and just being safe in general.

It can spoof System Calls, PSID's and much more. Definitely something worth looking at if you want to be on the safe side of modding your PlayStation 3 Console.

Jumping in with a curius question.
Is psnpatch all you need to play online? Currently on 4.46.
