Either 3 things
1) you didn't screw the clamps on tightly for the FAN: You must login or register to view this content.
2) you don't have adequate amount of thermal paste on the chips
3) the chips aren't on correctly on the heatsinks

The fan is spinning fast cause the ps3 chips are heating up and the ps3 is shutting off due to chips overheating!
