If you're using Control Console, make sure that it's actually installed on both your Computer and the PKG is installed on your PS3 Console. If you've done that and you're still having problems then simply uninstall it and then install it again and everything should sync back to normal.
the tools you are using are probably sending wrong values to the game offsets so it freezes your console. you can unfreeze it if you use a debugger and if you go in the dissasembly tab and press f5 then delete
