I tried to make this as simple as I can.

UPDATE: I'll add how to add a patch to a pkg and pack it back together. As it seems no one knows how to do this. But it is fairly simple.

Well
Alot of you have been having trouble with juddy's tutorial.
He states that it's not for "noobs" so I'll show you guys my way.
Everything you need will be in the download that I provided.
EXTRACTION
Step 1) Extract everything from the folder provided. (Downloads at bottom of page)
Decrypting/unpacking
Step 2) Find something to decrypt/unpack.
If your going to decrypt a pkg, then unpack it, open up tools-->signed decryptor and decrypt it.
After you are finished with that, either open up tools and run ps3gtb.exe and drag and drop the pkg file into the unpack pkg box, open up release and run PaCkAgE gUi.exe and use that.
I recommend using ps3gtb as it's easier to use.
Leftovers/editing
Step 3) You should be left with a folder that contains a few items.
(param.sfo, ICON.png, EBOOT.bin, USRDIR.
There might also be a few other files in their, so don't worry about that.
You can edit the ICON.png, but make sure to keep the size the same.
You can add sound, just look up the tutorial created by DT.
All sounds must be in AT3 format. Once your done editing, it's time to repack.
Building the pkg
Step 4) Have your folder with everything in it on your desktop.
Open up PKG builder and select your EBOOT.bin file.
Hit done. Verify that the game version is correct by using a SFO editor, then hit done again.
Then, it's time to modify the EBOOT.bin. You can either do this manually if your smart enough, or just make it easy and press build. If you get errors, retry it, and if you still get errors, you must modify the EBOOT.bin by your self.
If your that far with no errors, it's time to build the PKG file. You can either select 3.41 CFW or 3.55 Select the one you want, and press build.
A PKG file should be available in the PKG builder folder now. It will be named after the content ID.
Finalize/sign the package
Step 5) It's now time to finalize and sign the package.
Open up Release then run signer GUI.exe browse for the newly made PKG file and hit the sign button.
(Make sure that all files are in the folders they were in, or THIS WILL NOT WORK!) You are now finished.
Testing/installing
Step 6) You can now install and run your PKG file on your PS3.
Heres a test file for you to use, I signed this last night.
Some of you should remember this, it's the OLD FTP server we used to use back when we used 3.41 jailbreaks.
