Maybe your HDD its broken ?? maybe try change hard drive

Do a full restore on the PS3 and if that doesn't work, ring sony

Replace your HDD and have your firmware ready on a USB Stick to re flash your current firmware when installing new HDD.

I have the same problem, I had to buy a new PS3

Try to choose Option 4 in recovery mode [Rebuild Database]

If that doesn't work, try Option 3 or 5 (you said you restored but I'm not sure which one you did)
