Do I need to create a folder on the flash drive called PS4 then create a folder inside the PS4 called DATASAVE then what please ?

To setup your USB Flash Drive, I recommend just copying anything game save you currently have from your PS4 to you USB as this will add all the correct folders. If done properly the folder older should look something like this

Like Raging said just copy any of your saves to your USB and then when you already have your profile set up on Save Wizard just resign the save to your profile and it will automatically be put in the right folder
