You can earn it back but it doesn't seem to last more then a round or two. Is this a glitch on Treyarch's part or what? Quick Revive Perma Perk is still working. It doesn't make that big of a difference but I thought you should know.

UPDATE: Some people have lost Permanent Quick Revive. This is all apparently a glitch.

this happened after the game settings updated to 22.43.11 from 22.35.5, i lost the metal bars and headshot one and when im in a game of zombies every time i get 2 headshots with 1 bullet a green smoke comes up but i dont unlock it anymore, its locked to unlock, you cant get it... they need to fix it along with a million other ps3 problems
