Hellooo and I'm going to HOPEFULLY teach you how to add buttons to a realistic mod menu. FIRSSST you need to make a COD4 Mod Menu and get name your menu (i.e. ^4Admin) Now for the tutorial!!
First you need to download Hex Editor. Here is the link-->
https://download.cnet.com/HxD-Hex-Editor/3000-2352_4-10891068.html Now that you have that, open up your GPAD0_MP (assuming thats where your mod menu names are) with Hex Editor. Now on the right look for where it says ^6Admin or what ever you put as your menu name.

Right after the last letter in your menu name you want to put the following numbers for the corresponding buttons.

0E = Start
0F = Select
10 = L3
11 = R3
12 = L2
13 = R2
05 = L1
06 = R1
14 = D pad Up
15 = D pad Down
16 = D Pad Left
17 = D Pad Right
01 = X
02 = O
03 = []
04 = /\
