For the title that looks like an eraser, I think you have to reset all your stats via Prestige Awards.

There is the Nuked Out title which is from a 30 gunstreak in FFA

Like mentioned above, the 'Nuked Out' title is similar to the 'Nuclear Killer' title, except you have to get a 30 Gunstreak in Free-4-All. You're also not allowed to use any killstreaks that get you kills, so you can only use the UAV, C-UAV, VSAT and EMP.

i seen a few tweets from vahn, stating there was 3 hidden titles in blops 2, unless he was just trolling?

i have seen the eraser title, that would make sence to be rewarded that after stat reset

and Gun master for mastering Primaries and secondaries
