I don't know how to fix it and suggestions would be appreciated. I've also tired formatting my xbox and deleting the update and redownloading and still got the glitch/error

sounds like a gpu error. prob caused by overheating. black ops 2 puts the gpu under a lot of stress. try keeping your system cool.
