You wouldn't spawn on opposite sides of the map, you would probably get dropped in several hundred meters away, and you wouldn't all 'spawn' at the same time, one team would already be there, and you would know the 'map' off by heart, and they would have civilians helping them out, because it is their native country, while the other 'attacking' team would have the civilians trying to slow them down, and they would not have a clue where the hell they are, as they have not lived in this 'map' for all of their lives

Dude, I know a lot about real warfare.
The goal of this would be to make a gamemode that eliminates every unrealistic aspect of MW2 as possible.
Not knowing the map by heart, not spawning at the same time, or not being able to respawn at all would be impossible for a successful first person shooter game.
By completely realistic, I don't mean completely take out the spawn system. If that happened, you would possibly spend 20 minutes riding in a helicopter or going on foot to get to the battlefield, just to be sniped on entry and never return.
You make some good points, but keep in mind that the point of the game is to have fun. Making the game realistic in every possible aspect would kill that point.

The army doesn't have infinite air support. Maybe you would start with airstrikes or chopper gunners, but you would have to use them sparingly.

Your idea would be pretty cool though, as a different game mode. I've thought about it before, but not much.
There would be one game mode that you could only play once. It'd always be on one, huge map, around the size of all of the maps put together. Different terrain, depending on which direction you go. There may be a city to your right, but an open field to the left.
(With anyone concerned with the technical aspect here, the map would be streaming, like single player, so that the part of the level you start at loads, followed by areas farther away from you as you're playing.)
You would ride in a vehicle to your camp, where you would set up camp, and ready your weapons.
You would also need food and drink in this game. Eventually, if you survive for awhile, you'd have to return to camp to eat/drink.
This game mode would be going on twenty four hours a day. Probably with 50-100 players on each side.
(Again with the technical aspect: There would be 20 co-hosts in this game type. Each host is the host for five players; from there the hosts' connections communicate with each other.)
If you are killed, you die. Permanently. You get to play the game mode once and once only.
Let's just say that nearby air support is being used in a battle moderately far away, and is not able to be used in this battle to keep it fair. (Yes that's semi-realistic, it happens in real life.)
About the heartbeat sensors: The technology doesn't exist in real life, but because of the size of the map, few people would use it anyways.
