Theories

1- HDD method. ( Alot of people said it's fake and I believe this)

2- Proxy: Mainly it's the main method to get patch back on your PS3.

3- Linux ( Little information on this. Patch is edited using linux on PS3 , then you get it back on PS3 using proxy)

What if you get programs like Simple DNS Plus and Apache, and redirect your PS3 to a patch_mp.ff to download to your hard drive, like you downloaded items in Playstation Home?

Okay here's my theory... It's just like the old Phaze hack where you enter the cvars onto the usb stick. Even though IW downloads the game settings(which overrides this method), I think wireshark can pick up the packets when the downloading game settings stuff is occuring. You find out what IP address is sending the information and you spoof the IP. You then Inject your own mp files with cvar codes in them.

This isn't really related to modded lobbies, but user hacks in general.

but instead of using a ps2 disk, trick the system into thinking that the MW2 disk is inside the system and then install the patch?
