Maybe because I've completed those challenges and don't use it anymore, but a game ending streak that causes people to camp to aquire has fundementally changed how this game is played.

I would definitely take out painkiller which would make life easier.

Off topic : If i were to take one perk out of the game and into real life i'd go for commando pro. Imagine jumping from the highest building just to realise you're not dead. PWNAGE RIGHT?
