This has never happened to me ever even whilst bypassing.

i never had it while bypassed only that one time using the patch 1.11

It's probably just a glitch where the level fastfile (mp_terminal.ff, etc.) doesn't load correctly and then runs an error. Kept happening to me trying to join a game once.

Yes I have.
Once it was when I bypassed and joined a Buddy.
It said: "Your fastfile is different from our Server."
Meaning the patch was not similiar to my friends.

quite possible as the map loaded fine when you see the screen shot the white bar was full, then the screen went black but my friends where in there and no one i know has EVER had this beofre
