I was thinking of just editing the current customized patch I have, what programs do I need? what does it take? what is the procedure?

If you decompress and all that you can custom code it yourself with anything you want in it. Thats what half of the Patch Editors do here on NGU.

I've followed this tutorial 4 times, it always screws up, and I'm following it very carefully, yet it just gets all screwed up...

patch "editors". hes looking to "make" a custom patch (im assuming from scratch). and there is one program out there, by pr0z i think? I have not tested it so im not sure if it works or not. But your best bet is to download the original patch off of your HDD folder, and editing that. Seeing as how its the "Original patch", with no modifications. + there is tons of scripts posted all over the forums, for you to use on your patch.
