heres what ive tryed & got no luck:
-delete patch & reinstall with modified one
-deleted save data
-put a different patch on
-tryed it out on both ftp & file manager
-disk formatted
-use dvars to reset it
-tryed every conversion(with regions)

Strangely enough, I had the same problem two days ago... I just turned off my PS3 and next day it worked...
Might be a different problem though, because usually I wasn't getting a black screen I was just freezing, not my PS3 just mw2...

i get that sometimes the only fix i could find was take a blank patch and put the gsc/cfg into the blank patch, worked for me anyway.
