knife throw accuracy is not dead on.

the cross hairs are clearly focused on his head but for some reason he pegs him in his left arm just below the shoulder.

he isnt that far away so will this mean that the knifes will be very hard to get kills with?

Yah it's called bullet drop/knife drop?

Also, just a little side note, knives replace your Primary Grenades. If you look at the bottom right corner you'll see a knife symbol instead of either a Mk 2 frag or a Symtex. Also, it "SEEMS" as though you only get one, however it is possible the player used one or more before OWNING FourZeroTwo.
