I can't host it now but I can tell you how its done.
If you have your menu in a file called buttons_default.cfg you just need to put it on the root of an USB stick and plug it in the USB slot closest to the eject button of your PS3.
Then you need the infection and start it in a splitscreen match.
(I think you know how to do that)

Things you need for a USB mod menu:

1. A USB Drive
2. Files for the USB mod menu (.cfg file/s)
3. A friend who can infect you with the mod menu activation

Ask the person who is infecting you what you need to do and how/where to place the files. The infection may differ from person to person.
