and found if you pull your throwing knife out and hold it and press and hold L2 and release R2
you'll still be holding it and able to throw it.
I know its pointless just thought i share.

So basically
1.Pull out and hold equipment
2.Press and hold L2
3.Release R2
Now throw it by releasing L2 instead of R2.

This does work with all lethal equipments
