This has been happening to many of my friends too, I have a temporary solution. If you have a game you really, really want to put in your vault you can record them in parts. This might take up a few slots, but if you really want to save the game, it's worth it. This is just a small tip/solution. It's not the best, but it gets the job done. I'm not sure if I'm the only one that gets it frequently because there are hardly any threads about this problem... So yeah, whoever is experiencing this problem, this is a partial fix. Also if you run out of room you can put your game in a friends vault if he lets you

I see what you mean here but what about those who can' even access the file? Like me, it gives the error the instant I hit Play. I still have them in my Vault though, just waiting to see if IW will ever fix it :/. Thanks for the post regardless.

Right, but you can access it in theater. Play it in theater and edit it in there Don't put it in the vault... If you play too many games and it's gone, it's gone for good... So edit it IN theater

Load it straight from THEATER do not put it in the vault and then edit and place it in the vault, get it?

Yeah, the only downfall here is that it takes up 2 slots, compared to 1... Also, you might have to speed it up, which I'm not a fan of because it kind of "ruins" the quality of the video if you know what I mean, you miss those epic parts when you have beast reflexes XD...

I know but it was old and I didn't know it didn't work. I didn't wanna leave me party to edit but then next thing you know it's gone from Theater I hope IW can find a solution to this.. I had a long killstreak in under a few seconds :/
