Make sure you are running on a Wired Internet Connection by using a KTA-45 Ethernet/Lan Cable onto your router.
Make sure you rebuild database on your PS4 to clean any files you dont need or refresh it.
Reset your router for 10 minutes fully unplugged with your PS4 turned off.
Make sure your disc isn't scratched.
