Then Select "Downloads"

Ive only used Window Blinds, and DesktopX.

Windows Blinds is good, but I noticed it occasionally would prevent me from being able to operate a toolbar in a particular program. (Microsoft Word.)

DesktopX is in my opinion AWESOME, it allows you to completely custmize the desktop, with "widgets" (add-ons) such as calenders, weather updates, etc...
Bonus is they are free!! Theres a ton of themes to choose from a select handfull do charge for the theme, but majority don't.

I'll give you a link to it so you can use it forever, just PM me because I don't think warez are allowed here.

Also use the uxTheme patch if you want to use themes without alternate programs to apply them for you.

Heres an example of what the desktopx interface looks like. You have no access to your desktop icons. You now have to go through "my documents" and then open "Desktop"...

but heres what it looks like...
