ALS is a motor neuron disease. It is characterized by muscle spasticity, weakness due to muscle atrophy speech impairment and difficulties in swallowing and breathing. The idea of using ice water for this was for people to briefly experience the muscular difficulties that ALS patients suffer from. It's nowhere close to what they experience, but you get the point.
