It would be helpful if you included what cfw your on what SPRX + EBOOT are you using modded update rpf?

There is many reasons for this the most common being that you are using the wrong region version of GTA, if you have a blus copy and are using a bles EBOOT, if you converted blus to bles this may have not been done correctly and has become corrupted you may also have corrupted files in your SPRX/EBOOT/RPF and would require a fresh download and reinstall
