First off open up photoshop, the go File>New choose the size you want it i used 380x130pixels and make sure the background contents is white.
Next you want to hit "D" to make sure you are using the default settings.Now go to Filter>Texture>Grain and apply the following settings. Also i'll write the settings if you can't read for some reason.
Intensity 100%
Contrast 100%
Grain type Vertical

Next go to Filter>Artistic>Neon glow and apply these following settings.
Glow size 5
Glow Brightness 20
Glow colour: Green as you can get !

Then go Filter>Stylize>Glow edges and apply these settings..
Edge width 1
Edge Brightness 5
Smoothness 1
