1. An avatar
2. Signature
3. YouTube channel background
4. YouTube Intro.

It really all depends on who's doing it. Some people will do it for vbux and others will do it for paypal. The quality between artists also differs a lot so I think you should take a look at who's currently taking requests and check their previous work to see if you like it. Most people will put the price list in their request thread as well so you know exactly how much you're paying.

Unfortunately no I don't. I haven't been active long enough on the forums to get a good grasp on who I think is the best. Just take a look in the auctions and shops section and you'll find something you like
