Heres how I did it:

I started off re-creating the base.I made my own rounded-edges rectangle,followed by making a new canvas that was 5px by 5px,and finally,using the pen tool and a stroke,re-made the caps on the upper left,and lower right ends:

Then,I asked around and found the font that was being used,So I made my own text by putting spaces between the letters and then rasterizing the text layer and control+J the different letters so I could manipulate them.I created two straight line shapes,added an outer glow,and cut them down the middle to only leave the upper/lower half.This is my final product:

Sorry,I was contacted by the owner of the original and he asked me to please not release the base to anyone.He was also surprised to see someone that could copy it xD

Just switch the little neon part around and it wont be the same ^^
