You do have to register on the forums to play, but that takes about 2 minutes. There is a client download, and it IS VIRUS FREE. I have scanned it.

Virus scans do nothing on a runescape server as they only find viruses in .exe files and runescape servers have none of them (usually), if there was a virus it would be coded into the java files.
