Gta IV is the worse GTA title ever. It was ported from XBOX version and they did it pretty bad. I think it was the most demanding PC game at the time it was released. Even now, 6 years later, you need pretty strong gaming rig to be able to play in 1920x1080 with full antialiasing... Anyway, try upgrading your GPU, but don't expect a miracle, unless you get yourself a beast like ati 7970 or nvidia gtx 580...
