I used the following video and got this achivement on my second or third run through.
I will provide some pro tips that I used...

[*=left]PLAY ON EASY!
[*=left]This means you should play through on hard, etc. Get through all the missions in the Story Mode and come back and replay this, as playing it on Easy will reset your difficultly mid story mode if you are not finished with the story mode.

It's that easy.
