I had same problem, go to multiman and have black ops 2 in. Press triangle on black ops 2 and go to update and it should download a .pkg file on your usb if its plugged in, then just install the .pkg Thats what i done and it worked

What 4.21 cfw? You need to be spoofed higher than 4.25 and need to get the "eboot fix" for 4.21 cfw. I just have Rebug 4.21.1 installed, went to the rebug toolbox, set the ps3 to rebug mode, installed the newest multiman which automatically fixes the eboot for you.
