I had same problem. Well go to sonyentertaimen site. log in with your ps3 sing in and go to manage devices. u can deactivate all 5 consoles at once. just remember activate again on ur ps3. Remember you can only do it once in a year.

umm it not once a year it once every 6 months

then u have 2 give them details of old PS3 like serial number and all those little details and a good reason Y u deactivating and y u cannot do through the website + proof.

No u dont. if u go trought entertaiment site it have all details on all ps 3 u have logged in with ur account. So dont need any numbers or proof. if u deactivate all 5 we say so thats it.. they r gone.
